How does it attach to the toilet, does it just rest there?

It's very easy to attach the handles securely to THE TOILET.....remove the existing toilet seat and set aside for later....the handles come with a flat, pre-drilled metal plate that attaches using the holes (where the toilet seat attaches to the toilet)..... after the flat metal  plate is lined up with the holes lay your toilet seat on top and attach as usual .....insert ADJUSTABLE handles into each side of PLATE  making sure they LOCK in place ....... with the toilet seat UP......lower handles so the long protective plastic tabs (these cover the ends of the metal handles) rest on each side of the BOWL....and lower seat.
 VERY SECURE....HIGHLY RECOMMEND!!

Will this work with an elongated toilet? How do the open brackets stay in place?

Apparently, people are inclined to answer whenever a question about a product appears in their inboxes and don't really bother to read the question.
The open brackets refer to the FRONT brackets that seem to sit on the rim of the toilet with no means to secure them. Pads? To keep the metal from sliding around? They have to be cleaned periodically, so they cannot be glued to the toilet rim.

At the back of the toilet are two attaching bolts holding the standard seat and lid to the toilet. Elongated or round toilets have the same system UNLESS they are the 'quick release' or 'easy off' seats that have a different attaching system, not the standard bolts. Mayfair / Bemis and Kohler make such toilet seats.

Question 3

How tall are the arms? Very important for leverage.

About 10.5" (hard for me to measure). Check the specs online at Amazon. I'm 5'3" and have no problem with leverage. I do know that the support rails are the best I've found. The set I had before them cost four times as much and rusted out after about four years. The Carex have not had a problem so far.
